Enrollment Materials and Forms
These forms are to be completed and returned to your 4-H club so that the organizational advisor can submit all members' forms to the Extension office by April 1. Please check with your advisor for the date that they need the forms returned to them.
Member Enrollment Form is being updated for 2009. Please check back later. This form is required of all new members joining a Licking County 4-H Club. Due April 1.
Across County Lines Membership Request Form & Policy - This form is required of all youth joining a Licking County 4-H Club who do not currently live within the county. It needs to be submitted only for the first year of across county lines membership.
Horse Permission to Participate & Code of Conduct - All members taking horse projects, including Horseless Horse, and their guardians are required to sign and return both of these forms. Cloverbuds in horse clubs are not required to submit these forms.
Horseless Horse Mentorship - Members taking the Horseless Horse Mentorship project, their mentors, and horse owners will need to complete this Contract of Understanding. Form is due May 1 with Horse ID paper.
